Our Mission: To provide comprehensive, affordable, personal care to all of our patients; without regard to income, employment, insurance, faith, or documentation status. ¿What is the Membership Care Program? The Membership Care Program is new way to practice medicine. Our patients pay an annual flat that allows them unlimited office visits for a year, and discounts in labs and medications. This model allows the doctor to create a relationship with the patient through longer and more personalized consultations. The membership program is not medical insurance and is available for people 7 and older. Services Services • Physical Exams (wellness, sports, school) • Management of chronic diseases (diabetes, high blood pressure, high cholesterol, asthma, stomach reflux, allergies, migraines) • Appointments for common diseases (colds, skin rashes, headaches, fever, back pain) • Off-site referral for specialty care and coordination of care. • Patient Education
