Stellar Health Services, LLC is a mental health targeted case management agency based in Broward County, Florida, serving the mental health Medicaid population in need of social services / medical services. Our agency was founded a licensed clinical social worker, qualified supervisor, and certified behavioral health case management supervisor with over 10 years of experience providing services to varying populations in south Florida. Our targeted case manager(s) are carefully selected based on experience, knowledge of community resources, and dedication to the community that we serve. Our goal is to provide services to eligible Medicaid recipients that will help maintain/improve their levels of functioning, assist them to become increasingly self-sufficient, and connect them with the resources they need to do so. We realize that every individual is different, and has different needs; we aim to provide nothing less than high quality, individualized care/services.
