Sublime Care

Sublime Care is a registered NDIS and CHSP service provider, offering a range of services in the Upper Hunter, Lower Hunter, Newcastle/Lake Macquarie, Central Coast and Sydney regions of NSW. Our support are tailored to assist participants to live safely and comfortably at home and to actively enjoy their community. Our services are person-centred, holistic and responsive to the needs and goals of our participants, their families and carers. We listen to our participants and, in partnership with them, we tailor programs and support that ensure they receive the types of care that they want, where they want it, when they want it.
