Hospice care is a specially formulated team - doctor, nurse, social worker, spiritual care coordinator, home health-aide and personal volunteer-working together to treat your physical, medical, spiritual, existential, financial and family's needs in your home. Hospice is covered through the Medicare benefit for patients with a life expectancy of six months or less. We step in when you are ready to transition out of the cycle of aggressive, disease-centered treatment that can often be painful in the hospital. Our nurses are on call 24-7 and we transform your home into a safe, comfortable environment. Our team makes routine visits that provide a safety net of support for you and your family. With hospice, our goal is to provide you with immediate support as we work to alleviate the fears or anxieties you or your family may have. We will partner with you to create a plan that minimizes symptoms and controls pain so that our journey is as comfortable as possible. Our unique program provides you with ways to celebrate your life and preserve your legacy to pass on to your family and community. We believe this can be a time of reminiscence, celebration and above all dignity.
