Program of All-Inclusive Care for the Elderly **The PACE approach** Your care is provided by an Interdisciplinary Team (IDT) that is assigned to each participant consisting of the following staff: • Physician • Registered Nurse • Home Care Coordinator • Masters level Social Worker • Occupational Therapist • Physical Therapist • Recreational Therapist • Registered Dietitian • PACE Center Manager • Certified Nurse Assistant • Transportation Driver The IDT assesses each participant’s needs and delivers services based on a customized care plan.
