Sunwise Group

Sunwise Group is a care management company that supports older adults and people living with serious illness — along with their families, chosen families, friends. We offer in-person services throughout the Mid-Hudson Valley, and virtual services to all. Sunwise is led by Registered Nurse and Certified Case Manager Ellis Traver and Licensed Social Worker Mandy Osborne. Ellis and Mandy are trained professionals who prioritize client autonomy, independence, and well-being. We navigate complex healthcare systems, promote healthy aging, and help increase our clients' quality of life. Every person is unique, every family is unique, you are unique. We customize our services based on you and your situation. The ways we work together, and the frequency of how often we help, is up to you.
