Our Mission: Supporting families of the medically underserved with culturally sensitive and linguistically appropriate health education and assisting, our community members with dignified discussions on advance care planning and life affirming goals of care. Our Vision: Our vision is to become the primary educational resource for assisting those medically underserved members of our community with greater access to competent care and support for their families as the face the uncertainties in life transitions.
