The Heartcloud

Patients living with chronic disease often fail to follow care plans leading to troublesome and costly consequences. Sometimes it's because life gets busy, or perhaps they are actually feeling better, then one time they do something they should not, or they simply fail to keep on top of their clinical condition. Clinicians would like to keep on top of their patients, but the vast volume of data and business processes are simply too much. The HeartCloud is about passively capturing and managing critical clinical data with the minimal involvement of the patient. We provide the means by which to capture clinical data directly from patient's homes and apply advanced business rules to the clinical management of patients. Additionally, the HeartCloud allows clinicians to design and apply clinical rules against the data that arrives so that clinically significant information is called-out for specific follow up. The HeartCloud is the tool to actively manage patients regardless of where they live.
