The Kidney House has a mission to provide patients with diabetes, hypertension, and kidney failure a better quality of life by having the best Primary Care, Case Management, Disease Education, Diet Management, and Social Services in one convenient location. We work in collaboration with the Nephrologists and other specialists to foster patient wellness and satisfaction. We assist in monitoring and managing: Hypertension, Diabetes, Cardiovascular Health, Pain, and Mental Health. The Kidney House provides the highest quality of care to every patient. By doing so, we work to improve patient’s outcomes and reduce hospitalizations. Our highly experienced professionals work with patients to help achieve optimal kidney health.
