The Navigation Center – Integrated Whole-Person Model of Care The Navigation Center is built on a simple but transformative belief: housing is only part of the solution. True stability is achieved when we meet people exactly where they are and understand the full story behind their crisis—trauma, grief, medical conditions, mental health challenges, educational barriers, and the generational cycles that impact so many families. Our integrated model brings together housing navigation, social work, medical and behavioral health care, education partners, and community resources under one coordinated system. This approach eliminates fragmentation and ensures individuals and families receive whole-person, trauma-informed support from the moment they walk through our doors. We focus on understanding the underlying issues—not just the visible crisis. Whether a family is facing eviction, recovering from hospitalization, navigating mental health needs, or experiencing the effects of long-term instability, our team builds a personalized plan that addresses every layer of their situation. We work closely with MUSC Population Health, the onsite MUSC Cares Clinic for the Unhoused, onsite Mental Health partners, and Social Worker team at Charleston County School District to provide access to comprehensive services including medical care, mental health support, dental care, case management, and educational stability for children. Our whole-person approach creates long-term change—not temporary fixes. We walk alongside families and others through crisis stabilization, safety planning, budgeting, benefits navigation, employment pathways, and the transition to sustainable housing. Our model ensures that no family is left to navigate these challenges alone. The Navigation Center serves as a community hub of support, connection, and hope. By integrating services and building strong partnerships, we are breaking cycles, restoring dignity, and move them to lasting stability.
