The Oakleaf Group offers a range of specialist services designed to meet the needs of adult males with complex neurological conditions and brain injury. They may have associated complex cognitive impairments, challenging behaviours and/or physical disabilities including Percutaneous Endoscopic Gastrostomy (PEG) feed and tracheostomy care. Based in Northampton, our services provide: • Assessment and Specialised Rehabilitation • Continuing Rehabilitation and Review • Community Placements with Graded Support • Maintenance and Long Term Services • End of Life and Palliative Care • Respite Care (subject to bed availability) Our large team of healthcare specialist and effective inter-disciplinary working is paramount to the development and success of the individual programmes of care. Our on-site multi-disciplinary team include: • 24 hour specialised nursing (RMN, RNLD, RGN) • Neuropsychiatry • Neuropsychology • Occupational Therapy • Physiotherapy • Speech & Language Therapy • Rehabilitation Assistants • Family Liaison Team • Horticultural Therapy • Access to dedicated GP Service for all residents
