NPO that provides transition process, education of case managers, interdisciplinary know how transfer as to support young adults and adoleszent patients with chronical deseases and disablities. Growing up and facing changes in education, work place, living while they need a transfer from the paediatric doctors to specialists for adults has to be supported with a holistic approach. We cooperate with patients orgs, medical resources and social counseling as well as with health insurers and service providers. Our case managers are available to the patient during the transition phase (usually between 1 to 2 years) and if needed even furthermore up to 25. Else we engage with authorities and law makers in Switzerland.
