The Transitions Clinic Network (TCN) is a national network of community-based, primary care clinics that provide healthcare and social support to people who are reentering their communities after incarceration. Central to the TCN model are community health workers with a history of incarceration. Affiliated with the University of California San Francisco and Yale University, TCN manages its own two clinics in San Francisco and New Haven and provides technical support and training to more than 40 primary care programs in 14 states and Puerto Rico.
