Trellis, born out of MaineHealth, is partnering with employers across Maine to reduce their total cost of care through a network of advanced primary care practices. As healthcare spend continues to increase for employers, employees are struggling to establish consistently accessible, high-quality care. The Trellis model is restoring the patient-provider relationship to get people better engaged in their health and wellness in ways that the current healthcare system doesn’t allow. Our collaborative teams consist of a PCP, physical therapist, mental health specialist, pharmacist, and health coach to address employees’ whole health needs. We are on call 24/7 to answer urgent questions and triage members in need.
