For over a decade Trilema has been rigorously evaluated and proven to significantly improve patient care in 50+ European hospitals and scientific societies with important alliances such as AMGEN, Pfizer, Novartis, Siemens Healthcare… Under a SINGLE PLATFORM, we create different patient control and monitoring environments. We adapt the technology to control routes previously established by the scientific teams. Each environment allows the activation of routes for a COMPREHENSIVE PATIENT EMPOWERMENT, eLearning platforms, online patient-professional communication channels, remote home monitoring, virtual waiting rooms, appointment management, integration of medical devices and wearables. We developed the technology for the eHealth OnCommun project (Catalan Institute of Oncology), the most powerful European initiative in digital integration of Psychosocial Care and Health Education services for oncological patients: ICOnnecta't. Our technology engages patients, families, and clinicians to improve health care delivery. Our intuitive interface allows our system to significantly improve patient care by streamlining workflow and providing decision support at the point of care.
