TrustedCare was founded to solve the lacking requirement of collaboration between the population analytics and care organizations. In the era of value-based care, a comprehensive 360-profile of a member’s overall health along with associated co-morbid risks is necessary to ensure impactful care. Furthermore, this need for a comprehensive view is becoming a critical factor in delivering effective care for ACO and Medicare Advantage Plans, due to the increase in members with chronic conditions and the tsunami of the baby boomers in the coming decade. TrustedCare, has developed a personalized risk analytics and care management approach to address member’s clinical and non-clinical wellness challenges for members with chronic Cardiac, Pulmonary, Metabolic and ESRD co-morbidities. With this capability TrustedCare, facilitates providers and care management team in the creation and orchestration of member specific comprehensive care plans. In addition, working with its care network partners TrustedCare assist in addressing specific care and wellness needs. TrustedCare conducted a pilot with a MA plan and demonstrated its platforms capability to identify 20 percent improvement in the cost and wellness of their population.
