US PostAcute Service Solutions, LLC (USPASS) is a Network of Skilled Nursing Facilities that provides a comprehensive range of post-acute care services. Our premier facilities provide care to individuals who had an acute illness, injury, or exacerbation of a disease process. These individuals require less than an acute hospital level of care, but are faced with different levels of medical or rehabilitative needs. Each care is coordinated by an interdisciplinary team composed of physicians, nurses, social workers, and other relevant professional discipline. Our primary focus is the patient and their recovery.
