Utah Case Management

Utah Case Management (UCM) is a dedicated public resource company supported by Medicaid. We specialize in services for individuals enrolled in Utah’s New Choices Waiver program, Housing Related Services and Support, and Division of Services for People with Disabilities. We assist elderly individuals, those with disabilities, and low income/homeless or incarcerated individuals, by working with community and healthcare partners to develop support plans that meet each client's needs. Our goal is to help clients thrive, stay engaged in their communities, and achieve long-term stability and success. We believe that clients can reach a sense of self and self efficacy by getting involved in their community and using resources available to them.
