Vivify's Remote Care Management solution enables patient engagement and remote monitoring for a multitude of health conditions. Our digital health platform is designed to support communication between patients and providers, and to help increase care when the patient is outside of a clinical facility. Patient engagement and monitoring is accomplished with +HOME Managed Health Kits,+GO Mobile App and an interactive voice response solution (IVR). Vivify Health’s end-to-end platform uses a unique combination of cloud-based services and biometric accessory devices which are all administered by clinicians from the same Care Team Portal. Vivify Health is located in Plano TX. Our office is in a high energy location just above the best restaurants, wine bars and beer gardens in town.
