WadeHealth helps relatives stay connected with loved ones in post-acute care facilities, or receiving home health care. Loved ones receive regular updates on patient care and well-being through the convenience of WadeHealth's easy to use mobile app. WadeHealth peace of mind to patients, their family members and loved ones, no matter the distance between them. WadeHealth is a mobile app that has a two-pronged approach. First, it is communication device between providers and those that patients authorize to be involved. It sends notifications alerting recipients of updates to a patient's care or status. Second, it sends surveys out to users to score and let the hospital and post-acute providers be aware of a patient's status at various intervals. Hospitals receive the benefit of fewer readmissions, post-acute facilities can triage patients in the community and make more money by readmitting them for another short stay, and home health care companies get to bill for two episodes, as opposed to one episode. Patients become deconditioned after in-patient rehab stays in the community, and they generally are quick to readmit back to the hospital. Our mobile app catches those responses and triages their care and allows the nursing home and home health care companies to triage their care, hopefully preventing a hospital readmission.
