I am a Transplant Transition of Care Pharmacist at Ronald Reagan UCLA Medical Center in Los Angeles, California. Currently I help transplant high risk patients to identify and address any medication access barriers to ensure that they have their medications and are counseled on appropriate use after discharge in order to avoid and reduce re-admissions.
