About Ann Asmus Tourbier

I am a Director of Social Services at LindenGrove Communities, a leading provider of sub-acute rehab and long-term care services for the elderly population. With over 15 years of experience in this field, I hold a Certified Social Work certification and a Master of Business Administration degree, which enable me to combine clinical and managerial skills to deliver quality and compassionate care. My core competencies include psychosocial support, discharge planning, healthcare management, and rehabilitation. In my current role, I also apply my certifications in Challenging Behavior and Dementia Practitioner to address the complex and diverse needs of the residents with cognitive impairment and behavioral issues. I work with multidisciplinary teams to develop and implement individualized care plans, interventions, and referrals. I coordinate with external agencies and providers to facilitate the placement and home health services for the residents who are returning to the community. My mission is to enhance the well-being and dignity of the residents and their families, and to contribute to the vision and values of LindenGrove.
