About Ariel Hammock

Licensed Social Worker (LSW) with 8+ years of experience in case management, person-centered planning, and care coordination. I specialize in advocating for vulnerable populations, navigating complex healthcare systems, and connecting individuals with the resources they need to thrive. Currently, I serve as a Health Care Coordinator with Centene–YouthCare, where I conduct health risk screenings, identify social determinants of health, and collaborate with interdisciplinary teams to deliver individualized care plans. My background also includes child welfare, medical social consultation, and psychiatric rehabilitation—giving me a broad perspective across healthcare and social service settings. With a Master’s in Public Administration and a Bachelor’s in Social Work, I bring both clinical expertise and leadership readiness. My goal is to transition into a leadership role in social services, healthcare, or long-term care administration, where I can build strong teams, improve service delivery, and create lasting impact.
