About Bridget Lukaszewski

Over 8+ years experience in case management, providing support to families with children who have special needs/behavioral concerns and coordinating care to ensure services are being rendered effectively. I currently am a Lead Care Coordinator at Malvern Community Health Services. My duties include covering a caseload of clients, supervising/overseeing the work of care coordinators, providing staff development/disciplinary actions as needed, recruiting, hiring and training new staff, coordinating interagency meetings with managed care companies, schools, and other agencies, completing social determent of health screeners with families to offer additional resources and meeting deadlines to ensure clients stay eligible for services. Previously I worked as the intake specialist and was responsible for maintaining communication with schools or referral sources, completing the initial intake and setting up families for IBHS services. I have worked in the mental health field as a TSS and Job Coach, providing interventions and services to individuals on the autism spectrum as well as other various mental health disorders.
