About Bushra Islam

At InnovAge PACE, I enjoy working with seniors providing MNT in a community setting. This state of the art facility provides medical care, skilled therapies, recreational activities, nutrition/dining, social and behavioral services, dental and transportation. As an important member of the IDT, I create, review and update the care plans regularly to ensure nutritional needs are met while improving their ongoing medical conditions. Many participants in Richmond area are food insecure and unable to prepare food by themselves, so they are being referred to food assistance programs such as meals on wheels and Mom’s Meals. For those unable to get all their nutrition from diet alone and have increased nutritional due to dialysis, malnutrition, wounds etc. are ordered nutritional supplements to improve their health. My previous experience at Vida Health as remote dietitian included diet consults and frequent follow ups using an interactive app that keeps track of my members progress including their blood sugars, weights, eating habits, and physical activities. I loved providing the Whole Health program which not only focused on the physical health but also on the emotional health. My past experience at VA Medical Center includes- nutrition screenings, consults and reassessments to veterans with various medical problems at inpatient and outpatient settings - group classes on diabetes, weight management, and general nutrition -using CPRS, VISTA, and TMS for electronic charting, food preferences, and work related training.
