About Candice Kirk

As a Social Work Care Manager at Torrance Memorial IPA, Medical Group, Ambulatory Case Management- I have been privy to experience complex case management, along with care coordination amongst a multidisciplinary team consisting of nurses, doctors, medical specialists, gerontologists, a supervising LCSW, psychologists, physiatrists, and many other helping professionals. I provide emotional support, navigate eligibility and benefits of various health plans, social and county benefits, and provide community referrals. My experience working at the Behavioral Health Urgent Care Center (BHUCC) has provided me with one on one skills working with members in the community experiencing acute mental health crisis. Under the supervision of an LMFT, I was able to gain comprehensive knowledge on how to conduct a biopsychosocial assessment, work with a psychiatric team, and provide therapy for adults and teens. My educational background and experience interning at the CARE Program has given me a solid understanding of what it takes to work with individuals living with HIV in a medical setting. In order to adequately serve members with the comorbidities, and complex psychosocial needs as those living with HIV; I was trained to use evidence based strategies, and a wide range of care management practices. Furthermore, as a Licensed Clinical Social Worker, my scope of knowledge is based on a biopsychosocial perspective, and my practices are upheld by the NASW core values; service, social justice, dignity and worth of the people, importance of human relationships, integrity, and competence. My experience has given me the opportunity to assist in providing home-based, hospital-based, and in-patient based case management, conduct mental health assessments, link clients to social services, co-facilitate weekly support groups, one-on-one therapy, and work closely with a multidisciplinary teams to ensure clients receive thorough, exceptional care. Lastly, I have several years of experience working as an advanced behavioral interventionist utilizing applied behavior analysis strategies with children on the autism spectrum and their caregivers. The field of social work and mental health are quite necessary during these difficult times, and I am grateful for the opportunity to serve our communities and individuals in need.
