About Cassandra Herrera

I’m currently a care manager for Independent Living Systems, a care management organization contracted with various health plans such as Kaiser Permanente, LA Care, Blue Shield Promise, Anthem and Health Net. We provide enhanced care management (ECM) which is a Medi-Cal benefit designed to provide coordinated, community-based care to high-need Medi-Cal members with complex health and social needs. ECM is a key component of the broader CalAIM initiative, which focuses on transforming the Medi-Cal delivery system. When I’m not working, I’m attending class at Azusa Pacific’s Master of Social Work program, specializing in clinical practice. My goal is to become an LCSW, supporting individuals facing severe trauma and mental health challenges, that may be impeding daily tasks and well being through a person-centered philosophy. My passion lies in working with adolescents and families, backed by a diverse background that equips me to assist people from all walks of life. Driven by a fervent desire to be a champion for those in need, my goal is to guide them towards independence, fostering their health and wellness along the way. 🫶
