About Cortney Gordon

With over seven years of experience as a social worker in the health care setting, I am passionate about providing quality care and support for patients with disabilities and serious mental health needs who receive life sustaining dialysis treatment. As a master's level social worker II and a Titan advisory group member at DaVita Kidney Care, I represent the Titan division social workers based on my superior professional and leadership skills, and I provide feedback, support, and assistance with various social work initiatives and culture within my service area. I also facilitate a counseling pathway program specific to DaVita Dialysis, utilizing skills and interventions from CBT, DBT, motivational interviewing, and other theoretical frameworks to help patients with disease management, renal goals, and quality of life concerns. I actively participate and collaborate with interdisciplinary team members to assess, address, and intervene for patient needs and outcomes. Additionally, I sit as a board member for my local county transportation advisory board. I also have multiple certifications in leadership and positivity from LinkedIn, and I have strong skills in nonprofit organizations, fundraising, program development, grant writing, critical thinking, and care plan development.
