I absolutely enjoy service, coordination, and case management! I connect with individuals and inspire them to live the lives they want to live. I listen to their needs, wants, desires - and help individuals achieve their goals. I also hear people's pain and feel their barriers - and help to remove them. I know how to advocate for my participants and get them what they need. I am a resource-knowledgeable and recovery oriented Service Coordinator with experience in Managed Care and Social Services. I focus on helping disadvantaged individuals live the life they want to live. My experience is not over in case management, but also includes mental health recovery, addiction recovery, Project and time management, and EMR usage. I have Helped individuals navigate PA's long term services and support systems and have helped transition individuals from facility living to living in their homes. I pride myself on my ability to follow up with providers to make sure that my participants needs are being met or exceeded, as well as in my ability to keep my managers and other team members current of all important things involving my clients.
