I'm a strategic nurse leader with 20+ years in healthcare and over 7 years transforming care coordination into high-performing, patient-centered population health programs. My leadership approach combines data-driven decision making with a deep commitment to quality, equity, and team empowerment. At Wentworth Health Partners/Mass General Brigham, I led the successful integration into Mass General Brigham’s Integrated Care Management Program (iCMP) program, successfully meeting all performance benchmarks. I’ve designed and scaled cross-functional care models, collaborated with behavioral health and pharmacy teams, and built the kind of infrastructure that helps patients—and teams—thrive. I hold an MSN in Leadership in Health Systems Management and certifications in case management. I’m passionate about helping organizations advance their population health strategy, reduce disparities, and deliver sustainable value-based care.
