Experienced Senior Case Manager with a demonstrated history of Community Services industry. Working with Aboriginal, CALD. Mental Health Support,Education and community based Experience in Disabilities,Holistic Therapy & Counselling knowledge. Demonstrating extensive history and experience working to support people with challenging needs & requirements. Promoting and engaging, educational pathways whilst identifying individual health & wellbeing plans & goals for children to safely live in the care of their parents or families- As a community, social service professional who is highly educated in the roles of: Case Managing, Mentoring and supporting Aboriginal people & families at risk. Identifying their needs and requirements to support their mental health and wellbeing, to keep their children and families safe so they can continue to live in a safe home environment, creating cultural safety- I demonstrate I have the capacity to work with people from a diverse community and background and am able to identify the required supports,mentors and services best suited for the individual or group- I demonstrate this when utilising my assessment and fact finding skills when I am required to identify the worries, strengths and goals and next steps for a positive outcome for the individual person, child or family. My experience working in the community service industry has assisted me to hone my skills and abilities to collect data, resources and communicate with networks when required- I have been told, by my peers and colleagues that I have exceptional, understanding of people’s needs and wants and can identify specific individual needs and requirements, and am able to work as a team member- I have demonstrated and been told my my peers that I am a patient calm person who is able to work well under pressure within a team environment, or when required in challenging situations. I am able to work unsupervised, with exceptional time management skills and can work respectfully as a team member. I enjoy a healthy mind,healthy body lifestyle, and as a mentor, I am regularly supporting and building cultural awareness and business leadership for individuals, or groups. Whilst promoting heath and wellbeing, educational healing initiatives- I am a punctual and presentable person. Who is empathetic, considerate and friendly person who enjoys nature, good food company, whilst caring for country, wild life, animals and the environment.
