About Deepak Dhawan

Working as internal medicine specialist, for over 10 years in hospitals & private practice, I realized how many unnecessary visits to the hospital emergency room & hospitalizations could have been avoided if people had timely access to their physicians. This prompted me & my partners to look into bridging gaps into existing models for continuum of care, directed to improve quality patient care and providing cost-effective health care by adding home visits by a licensed physician who can go to patient's home (including Board & care, assisted & independent living facilities) to evaluate patient as a whole with comprehensive medical examination, reconciliation of home medication, evaluating their home situation including psycho-social issues, discussing patient's wishes & end of life care, in comfort of their own home & with their loved ones around them.
