About Dung Pham

I am an Accredited Practising Dietitian, Diabetes Educator and Health Coach who works within a client centred model of care. I specialize in aged care,diabetes and Chronic conditions. In my private role I work with clients, their family and health care team to create a step by step plan that suits their needs, health condition and lifestyle. I had had training in motivational interviewing, ACT therapy and NLP and incorporate these into my dietetic practice. I am a strong believer in self management and passionate about empowering clients to take control of their own health instead of letting their health control them. As an aged care dietitian I help residential aged care facilities provide nutritious foods that meet the physical and psychological needs of their residents.I do this by working with facilitly/clincal managers, chef, staff the resident and their families to come up with a mutually agreed dietary care plan. I also work with facilities to-Maximise energy and protein intake in those with small appetites -Improve the dining and meal experience-Use a food first approach - Adopt food fortification to reduce malnutrition-Improve and strengthen nutrition and hydration policies and practices -Dietary advice for residents with special dietary requirements such as renal, IBS, Coeliac
