About Grace Hunter

As a primary care physician, I strive to care and advocate for individual patients while gaining a better understanding of the broader social, economic, and health systems barriers to good health. I am also the Chief Medical Officer at Miga Health, where we are building the future of heart health. I previously served as Medical Director of Population Health for Ventura County Health Care Agency Ambulatory Care, where I managed a grant to implement remote blood pressure monitoring in our Federally Qualified Health Care Centers. I also served as physician lead for Whole Person Care, a program under Medicaid's CalAIM Initiative to provide Enhanced Care Management to Ventura County's most vulnerable Medicaid enrollees, including those experiencing homelessness, high utilizers of the ER or hospital systems, and people with serious mental illness and/or substance use disorder. I graduated from Stanford University and the London School of Hygiene and Tropical Medicine, where I was an English Speaking Union Scholar. I completed an MD/MBA at Stanford University and Internal Medicine Residency at University of California San Francisco, where I participated in the Zuckerberg San Francisco General Hospital Primary Care Track focused on evidence-based primary care, health systems leadership, and health equity.
