About Heba Tallah Mohammed

PhD- trained health systems researcher with more than a decade of experience supporting and leading multi-site studies in the areas of long-term care, home health (HH), and digital health across Canada and the U.S. I have consistently aligned clinical research with operational goals, related quality metrics, and value-based care, particularly in the HH and skilled nursing facilities (SNFs) utilizing PointClickCare and integrated digital wound care modalities. My expertise relates to real-world evidence generation, AI tool validation, and impact/outcomes research. I have authored more than 20 peer-reviewed publications on clinical effectiveness, care quality, and health equity featured in leading journals and conferences such as Wounds, Int Wounds and SAWC.Career Highlights• Conducted large-scale outcome evaluations across more than 100 skilled nursing facilities and 59+ home health agencies, assessing the clinical and operational impact of AI-enabled wound care devices. Our findings demonstrated decreased healing times and more consistent clinician care; the findings were published in Int Wound J and PLOS Digital Health.• Co-led two major SNF research collaborations, integrating Swift (ChartPick) and PointClickCare data to evaluate AI impact. Results contributed to peer-reviewed publications and conference proceedings and product strategy refinement.• Conducted multi-sector health research initiatives on digital health, clinical outcomes, and system performance. Managed research design, ethics submission, analysis, and dissemination across academic, public, and industry contexts with a strong relationship and emphasis on applied impact and in support of value-based priorities.• Produced more than 20 peer-reviewed publications that includes comparative data for wound outcomes, clinician satisfaction, sector performance (i.e, wound clinics, emergency departments) and health equity and AI accuracy. Topics ranged from aging, implementation science and effectiveness of digital health evidence in populations with co-morbidities.• Presented findings at 10+ national and international conferences including SAWC, WOCNext, and NPIAP; recognized with a number of research merit and high-score abstract awards.• Cultivated collaborative research partnerships across industry, academia, Indigenous health services, and digital health platforms to support grant-funded and value-based initiatives.
