About Holli Hoffman

I have been a nurse practitioner since 2003 in psychiatric and primary health care. Recently, I completed the criteria for California NP Independent Practice in a Group Setting. I completed a Doctor of Nursing Practice degree in 2015. I am board certified as a psychiatric and primary care nurse practitioner. I am also a mental health therapist since 1987. In my current position, I carry out psychiatric evaluations and follow mental health patients, prescribe their psychotropic medications, and provide limited cognitive behavioral and psycho-education. I have provided primary care to many patients that did not have access to county primary care. I have treated general medical problems such as diabetes, hypertension, throat and urine tract infections, asthma, COPD, and headaches.I also have provided wound care, removed sutures, taken cultures, and applied unna boots In addition, I functioned as nursing director for three nurses and as a consultant to clinical staff. I carry out training in mental health topics to our staff when requested. I order labs and tests and interpret results. I chart on electronic records, and have written reports on patients for SSI and nursing home admissions. I reviewed group homes and hotels where patients lived for health and safety. I also have seen patients in nursing homes and private homes on occasion. I have many years of running therapy groups. I have several years focusing on addictive disease. I was part of an ACT team, and perform crises assessments and intervention as needed. I have precepted many nurse practitioner students and second degree RN students. I have worked with patients across the age span, both in-patient and out-patient. I have taught Biology at the high school level, worked in the prison system as a therapist and the jail system as a nurse, and managed group homes for developmentally disabled individuals.
