About Jason Lanzo

I am a Blended Case Manager serving Bradford and Sullivan County area. I am experienced with providing support to individuals suffering all forms of mental illness. I have a strong desire to help others and I possess the patience and understanding that are greatly needed in my line of work. Before starting my career in human services I was independently contracted through the County to provide support to individuals transitioning from state hospitals back to their community. Once I was officially employed through Bradford County human services I have continued to provide quality services to individuals to help them obtain a more satisfactory social, economic, mental or physical adjustment to their environment. I have been responsible on interviewing clients to obtain social histories; provide comprehensive information about agency programs; determine and verify eligibility for program services; provide referral services and negotiate on behalf of clients for referral services; and formulate and implement social plans of action. I possess the ability to manage a caseload, prepare reports, and keep well-organized records. Apart from my duties of targeted case management I also meet with patients who are admitted to state hospitals from Bradford and Sullivan County. I also facilitate over the discharge planning of these patients back into their community and sometimes to other counties. During the discharge planning meeting I am able to provide information to the treatment team in regards to resources and agencies that may be of benefit to the patient once discharged. I then serve as liaison between the resources/agencies and other community organizations and services as assigned. Along with my 18 years of experience my abilities include managing time effectively, a strong sense of responsibility and effective communication skills that make me an asset.
