About Jennifer Joffray

As a RN Clinical Evaluator at iCare Health Network, I facilitate the transition of patients from hospitals to short term or skilled nursing facilities. I collaborate with hospitalists, case managers, and iCare staff to assess the clinical needs and preferences of each patient and ensure a smooth and timely discharge process. I have a Bachelor's Degree in Nutrition from University of Minnesota and an Associate's Degree in Nursing from Goodwin College. I started my career as a clinical floor nurse at Touchpoints of Bloomfield, where I gained valuable experience in providing quality care to a diverse patient population. I enjoy working in a dynamic and challenging environment, where I can apply my skills in nursing, communication, and management to improve the health outcomes and satisfaction of the patients and their families.
