Convene Sauk County's Prevent Suicide Coalition and Housing Action Team while intentionally interweaving health equity and SDOH into all aspects • Prepare and conduct public health educational programs in a variety of community settings. Provide education to the public and clients specifically regarding: Prevention, Evidence-based programs, Emergency preparedness, and other departmental programming • Assess community health needs: collect, analyze, and report on local data, in partnership with community stakeholders • Conduct an equitable, data-informed and community-driven Community Needs Assessment (CHA) and Community Health Improvement Plan (CHIP) process in partnership with the SC Data Council, healthcare partners and the Ho Chunk Nation; prepare & present CHA and CHIP reports. • Serve as a resource on health promotion to the community and county. • Actively participate in defining community needs, identifying funding opportunities, and writing grants to secure funding for new initiatives. • Continuously evaluate coalition effectiveness; develop, implement, and evaluate coalition strategic plans. • Grant management for WI DHS PHEP program
