About Judith Cole

I’m a dual-certified Adult-Gerontology Primary Care and Women’s Health Nurse Practitioner with expertise in addiction medicine, primary care, and harm reduction for structurally vulnerable populations. My clinical foundation is rooted in low-threshold, community-based care across shelters and street settings, grounded in trauma-informed and harm reduction principles. My leadership experience spans designing and implementing treatment programs that deliver measurable impacts and managing cross functional teams that prioritize trust and dignity when working with individuals. As Program Director of Harm Reduction Mobile Health Services at Harlem United, I designed and launched low-barrier buprenorphine and hepatitis C treatment programs, lead interdisciplinary team development, manage grant-funded service delivery, and collaborate with research partners to track real world outcomes. I also bring experience in digital health through my work at Ophelia Health, integrating virtual care, patient-centered technology, and clinical product development. With a foundation in both street-level care and digital health innovation, I bring a unique perspective to solving complex healthcare challenges. This dual lens has shaped my ability to lead inclusive, data-informed strategies that scale access, improve outcomes, and drive system-level change.
