I have been in the healthcare field since 2007. I worked for 10 years as a CMA in clinic before returning to college to become an RN. Since then, I have worked as a Lead RN of the Care Coordination program since 2016. I manage care coordinators at 6 clinic sites. I am experienced in TCM (transitional care management), General Care Coordination, Health Care Home, Chronic Pain Care Coordination, Controlled Substance Care Coordination, Pediatric Care Coordination, MAW (Medicare Annual Wellness visits), IV Therapy, Medication Refills, Value-Based Contracts, Utilization Management, Pivot Tables, Microsoft Word and Excel, Epic EMR, Policy and Procedure Creation/Management, and much more. My team and I have excelled at decreasing cost of care to our patients while guiding them in the complicated healthcare world and also improving quality of life and health outcomes.
