As a cognitive systems engineer, I take a unique perspective on design, psychology, and engineering. My dissertation research involved 1) developing a novel accident analysis technique, the Systemic Contributors and Adaptations method (SCA), 2) analyzing a corpus of cases to identify patterns in how systemic pressures created goal conflicts and practitioner adaptations that moved systems closer to failure, and 3) proposing indicators of movement toward system boundaries that would allow practitioners in a system to proactively respond before resources were exhausted and an accident occurred.
