About Kim Walcoff

I am currently working for Elara Caring, a homecare company that provides nursing, hospice and therapy services to patients in their private residence. The patient has more than likely just been discharged from the hospital, or had an exacerbation of a chronic condition that requires monitoring and skilled services in order to return to prior level of function at home. I work with middle-aged as well as geriatric patients and strive daily to maximize their safety and function by training them in strength, posture, flexibility, balance and gait so they can meet their maximum potential. Many make wonderful gains. I work with many diagnoses from orthopedic injuries/surgeries and balance/gait dysfunction, to effects of stroke, cardiovascular and respiratory diseases, CVAs, transplants and behavioral diagnoses, MS and Parkinson's. I am certified in the LSVT BIG technique for Parkinson's patients and absolutely love it, as it employs my love for all things postural. I am involved in all facets of patient care, from beginning to end. It is my responsibility to submit all appropriate documentation as required per the guidelines of Medicare and respective supplemental insurance. It is important to create the most beneficial plan of care within insurance guidelines, and that patients understand the purpose, frequency, and necessary carryover with any exercise program provided when discharged. I also share treatment results and progress with all interdisciplinary team members, supervisors, doctors and caregivers as appropriate. I strive to have a cohesive relationship with these professionals and also with patients' families in order to provide the utmost quality of care. It is extremely important to me that I provide innovative treatments to maximize a patient's function, while also helping patients meet appropriate goals. I am a huge advocate of educating my patients and their families so they can understand their bodies, remain motivated and invested in treatment, and properly carry over exercise programs and techniques that they have learned in order to prevent decline. I visit patient's homes throughout the northern Houston area and have many in Conroe, Magnolia, Spring, Montgomery and Kingwood. In addition to my doctorate in physical therapy I have been working in the muscle/massage therapy field for over 20 years, performing corrective soft-tissue techniques for chronic stress and other tissue impairments.
