As a Care Transitions Coordinator at Enhabit Home Health & Hospice, I use my skills and credentials in Occupational Therapist Assistant and Psychology to facilitate seamless and safe transitions of care for patients and families. I work with a multidisciplinary team of nurses, therapists, social workers, and physicians to provide coordinated, patient-centered, and quality care across the continuum of health services.
