About Leigh Sand

With an educational background in Philosophy and Theology, and a varied path of life experience, skills, and travel I found my calling as a Hospice RN in 2016: at first as a hopeful volunteer during nursing school, then as a bedside RN, eventually the Compliance Coordinator and now the Patient Care Coordinator. I successfully helped revive our agency's Quality Assurance and Performance Improvement program and currently oversee our clinical staff and ensure patient care needs are met through both care coordination and direct patient care. I effectively helped double our average daily census within 6 months in my current position through team collaboration and making our referral to admission process more efficient. I love the interdisciplinary approach of hospice care, helping patients and their families navigate the social, spiritual, and clinical care needs of their end of life journeys. I am often struck at what a sacred opportunity I have to share in these moments before, during, and after death. I hope to continue to hone my interpersonal, leadership, and clinical skills in order to make the care we provide ever more meaningful, effective, efficient, and safe for my coworkers and our patients. I am fortunate to have found a special corner of the world in which to live, work, and pursue my career here in Southwest Colorado.
