As a licensed Registered Nurse (RN) with years of experience in the Healthcare and Preventative Medicine fields, I have a proven track record of building strong professional relationships within interdisciplinary teams and effectively leading teams toward better outcomes. I have gained the confidence and trust of physicians and their staff through my extensive medical and preventative knowledge and expertise.My passion for creating care management solutions for patients and their families has led to my success in my current role as a Complex Case Manager at Sharecare, Inc. In this role, I manage and coordinate holistic medical care for a caseload of up to 50 insurance members per week. I provide wellness coaching that includes mental, emotional, physical, and social determinants of health for the ideal management of chronic complex health conditions. I also create and write complex case assessments and care plans to address the holistic needs of insurance members, which has helped bend the cost curve for an Affordable Care Organization. Additionally, I coordinate care with physicians, labs, imaging, home health companies, patients, and insurance coverage to meet care plan goals.My previous experiences have enabled me to specialize in weight management, pain management, emotional regulation, behavior modification, sleep management, and positive coping techniques. I have successfully helped children, adolescents, and adults to manage stress and difficult emotions.As a former Case Manager at Virginia Hospital Center, I was responsible for leading daily care coordination rounds with interdisciplinary team members focused on discharge planning, and utilizing clinical knowledge to arrange services, provide access according to resources, and coordinate care amongst the complex medical patients. I have also served as a Cardiopulmonary Rehabilitation Director, Quality Assessment Nurse-Surgical, and Staff Nurse in various healthcare settings, which has equipped me with the necessary skills to excel in a healthcare environment.I hold a Bachelor of Science in Nursing from Samuel Merritt University in Oakland, CA, and am licensed as a Registered Nurse in the State of Maryland.
