Professional Summary: Over the past 3 years, I have been Hospice Clinical Liaison within a large hospital system in Richmond, VA. My Primary responsibilities are to provide coordination of care through facilitation, communication and discharge planning through the continuum of care. I provide information about Hospice services to potential Hospice patients and family, as well as provide Hospice education and support to staff. I am a self-directed independent worker with the ability to form strong relationships within the health care community and hospital. I provide care coordination for patient and family to return home to begin a successful home hospice admission and plan of care. My clinical judgement is trusted by physicians/providers to determine patient’s hospice level of care and most appropriate place of service. I am responsible to aiding in a smooth and time sensitive discharge, coordinating Home Hospice team, medications, DME and family training to adequately and safely provide comfort focused care for loved one in the home setting. If home setting is not a safe option, I admit patient and family into hospice services in the acute care setting of one of our hospitals, and provide daily support, education to patient family and staff while under Hospice. I am Certified in Advanced Care Planning, through Honoring Choices and frequently assist families with documentation of medical care wishes and Advanced Directives.
