Facilitate and coordinate long-term care home and community based services for participants.Develop and implement holistic ISP with each participant that addresses physical, cognitive and behavior health needs while collaborating with participants, guardians, and primary representatives. Complete annual care management interviews and initial intake interviews.Coordinate with outside agencies to implement personal care services, in-home nursing, in-home physical therapy, behavioral therapy, home modifications, medical transportation, non-medical transportation, home delivered meals, and specialized medical supplies. Ensure that participants are receiving appropriate services in accordance to ISP through monitoring calls, monitoring visits, and provider contact. Maintain accurate record keeping and documentation of all participants.
