About Michael Toft

As an Investigator at Maternity and Newborn Safety Investigations (MNSI), I lead investigations into cases of serious harm or death of mothers and babies in healthcare settings. With over five years of experience in this role, I have brought a high volume of investigations to timely and successful conclusions by planning, conducting interviews and analysing evidence. I have also contributed to improving the efficiency and quality of the investigation process by designing and implementing a casework management system, piloting and managing a casework triage process, leading internal quality assurance teams, and training caseworkers. Additionally, I have demonstrated my leadership skills by deputising for the team leader and managing the team when required. My background in public policy and management, as well as ombudsman and complaints handling practice, has equipped me with the knowledge and skills to resolve complex and sensitive disputes between the public and public sector or regulatory organisations. Before joining MNSI, I worked as a Senior Scrutiny Officer at the Professional Standards Authority resolving complaints about the organisation and regulatory bodies. Prior to this, I was a Casework Manager at the Parliamentary and Health Service Ombudsman, where I introduced and enhanced complaints management systems, managed teams, and wrote customer-focused replies and guidance.
