Completes the medical history, nutrition history, and related assessments for clients • Develops, monitors, evaluates, and revises nutrition care plans for high risk, and low risk clients and provides/documents nutrition and dietary counseling according to all local, state and federal guidelines Assigns/updates client food recommendations according to program guidelines// Documents client counseling and all other contacts in the medical record • Provides nutrition programs and outreach to health care professionals and members of the community as appropriate//Completes inventories as assigned • Provides classes and group nutrition education for clients and their families • Provides appropriate medical nutrition therapy based on the results of the diet history, diet recall, impedance testing, laboratory values, psycho-social factors, and additional medical information from providers//Individualizes the Plan for each client, including goals and a plan for follow-up • Conducts nutrition in-service programs for clients, staff, and the general public • Plans/holds a monthly breastfeeding class for individuals willing to learn more • Work disaster shelters for the aged in population, special needs, & general population
